Message to Survey Participant: Please read the information below before answering the survey questions. Your participation in completing the survey implies you have read this information and accept these terms

        Thank you for your interest in this survey. This survey is being used to gather important health information from the residents of Luzerne County.  The data will be used to develop web based health education programs for all residents of Luzerne County regarding the important health issues facing individuals living in the county.  The data will also be utilized to apply for health grants to improve the health of the residents of Luzerne County. In addition, this information will be made available to other health-related agencies in the County who will use the data to better address the priority health issues faced by residents. All you need to do is complete this online survey, which should not take more than 15 minutes to complete.

        ALL INFORMATION YOU PROVIDE WILL BE STRICTLY ANONYMOUS.  No attempt will be made to identify individuals that completed this survey    This information will be used only for purposes of describing the overall opinions of Luzerne County residents regarding the health related issues in our county.   Your participation is important and can make a difference.  Thank you for working with us to improve the health and the health care of Luzerne County.

INSTRUCTIONS FOR COMPLETING THE SURVEY:

1.      This survey is intended for residents of Luzerne County. If you are not a resident of Luzerne County, please do not complete the survey.

2.      You must be 18 years or older to participate. Please do not complete the survey if you are under 18.

3.      If you are unsure about how to answer a question, please give the best answer you can. Most questions have a “don’t know” option.
